What you will do:
  • Conduct strength-based reviews and reassessments, putting together reflective support plans focused on complex and high-cost care packages.
  • Collaborate with residents, carers, and families to identify areas of need and potential risks, recommending suitable actions or services.
  • Manage the continuing health care (CHC) process effectively, ensuring that all aspects are handled with expertise and care.
  • Work closely with a dedicated team to provide the best outcomes for individuals with complex cases.
