As an experienced clinician, you'll use your clinical skills and manage your own caseload of clients and provide clinical supervision to other case managers. The organisation supports clients in the North East and Yorkshire who have an acquired a brain injury, spinal injury or life-changing physical disabilities due to an accident or a birth injury.
- When a referral is received, you'll be responsible for completing a thorough and detailed assessment of their needs. We take a holistic approach so this will include their care, emotional and social needs. You'll implement a rehabilitation package and continually review and assess it, to ensure that it continues to meet the client's needs.
- Most clients are involved in a medico-legal process and you'll support their case through accurate report writing and continual re-assessment. You'll be able to put into place bespoke equipment, vehicles, holiday's etc. that will benefit the client's health and emotional needs.
- You'll work within a multi-disciplinary team consisting of nurses, OT's , Social Workers and Physio's and be able to share knowledge and experiences with others in the office.
- You will use your clinical and case management experience to provide regular supervision to case management colleagues.
- Must have a minimum of 3 years' experience within your field of expertise and 3 years' experience as a clinical case manager
- Full UK Driving License
- Professionally qualified and registered as a Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, or Psychologist. Case management experience is essential.
- Structured and logical thinking style that is comfortable operating both at a detailed level and at a directional level
- Excellent report writing skills
- Highly effective verbal and written communication skills
- Ability to build therapeutic relationships quickly and have strong interpersonal skills with clients and professionals
- Ability to work autonomously and also as part of a team
- Ability to demonstrate excellent time management skills and also co-ordinate others when working to deadlines
- Ability to apply new learning to work situation
- Ability to transition from your current role into the role of Case Manager
- Must be IT literate and have the ability to learn new systems quickly
- Strong problem-solving skills and the ability to manage difficult and at times, volatile situations
- Supervisory skills to support case managers in the organisation
