What you will do:
  • Conduct comprehensive CHC assessments, arranging funding care and liaising with health and care providers to ensure best practices.
  • Plan and coordinate with hospital discharge teams to guarantee timely and safe transitions to community or home settings.
  • Manage complex long-term cases including safeguarding risk assessment capacity assessments MCA and Care Act compliance with exceptional attention to detail.
  • Collaborate within multidisciplinary teams across health and social care driving innovation and excellence in service delivery.
Must have CHC or Hospital or Long Term Case experience.
