Job Type: Full-time, ContractAbout the Role:We are seeking an experienced Social Worker to join the Adult Team in Guildford.Key Responsibilities:
- Undertake comprehensive, person-centred assessments of need, risk, and mental capacity, ensuring outcomes are tailored to individual circumstances.
- Develop and review care plans that promote recovery, independence, and well-being, working in partnership with service users and their support networks.
- Deliver statutory duties under the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
- Facilitate access to appropriate services, including community resources and specialist mental health interventions.
- Lead on high-risk and complex cases, ensuring timely and effective interventions.
- Act as the Investigating Officer in safeguarding adult cases, coordinating multi-agency responses and managing associated risks.
- Provide expert advice and decision-making support to colleagues and partner agencies.
- Prepare high-quality reports for legal proceedings, tribunals, and multi-agency case reviews.
- Provide professional supervision and mentoring to less experienced social workers and other team members.
- Support the induction of newly qualified social workers and students, acting as a Practice Educator where appropriate.
- Promote reflective practice and professional development within the team, contributing to a learning culture
- Work closely with health, housing, voluntary sector, and other professionals to deliver coordinated care.
- Represent the Adult Mental Health Team at multi-agency meetings, contributing to joint decision-making and planning.
- Build positive relationships with partner organisations to enhance service delivery and outcomes for service users.
- Contribute to service development, identifying areas for improvement and implementing innovative practices.
- Ensure accurate and timely record-keeping in line with organisational policies and professional standards.
- Participate in audits, reviews, and inspections to maintain high-quality service provision.
- A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Substantial post-qualification experience in adult mental health services, including complex case management.
- In-depth knowledge of relevant legislation, including the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.
- Experience in safeguarding adults and managing risk in multi-agency contexts.
- Advanced assessment, planning, and decision-making skills.
- Strong communication and interpersonal skills, with the ability to engage effectively with service users, carers, and professionals.
