What you will do:– Conduct comprehensive assessments of children and families to identify needs and develop action plans
– Provide tailored support and intervention to children and families, enhancing their social, emotional, and physical well-being
– Collaborate closely with multi-agency teams to deliver coordinated and effective support
– Ensure all case records are maintained accurately and in a timely manner in accordance with statutory requirements
