We are looking for a dedicated Clinical Pharmacist to become a key member of our multidisciplinary team, collaborating with GPs, nurses, pharmacists, and pharmacy technicians to deliver high-quality pharmaceutical care. This role is ideal for someone who is compassionate, proactive, and self-aware, with excellent communication and problem-solving skills. You will work directly with patients to optimise medication management, support chronic disease care, and contribute to safer prescribing practices.Main duties of the jobClinical pharmacists are an integral part of the general practice team, working to improve outcomes from medicines while ensuring patient safety and value. In this role, you will consult with and treat patients directly, providing support for those managing long-term conditions, offering advice to patients on multiple medicines, and improving access to health checks.Key responsibilities include providing expertise in practical therapeutics, developing bespoke integrated care plans, and facilitating communication across the patients care pathwaylinking with primary care, secondary care, social care, and domiciliary services.
