  • Deliver comprehensive social work services, managing complex cases in the Adult Social Care team
  • Conduct in-person, complex assessments and discharge planning for hospital patients
  • Undertake mental capacity assessments and facilitate discharge planning meetings with medical colleagues
  • Provide support to families and liaise with medical and social care professionals
  • Mentor and coach new social workers, helping them to develop best practices
  • Collaboratively work with the entire team to manage risk and promote positive outcomes for service users
