- Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
- You will identify immediate health needs including, any external services or prescribing requirements that prisoners
- Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections
- Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.
- Continuous improvement of MDT working across HMP and healthcare teams.

- Registered Nurse- General with relevant NMC Registration
- Advanced Nurse Prescriber/ Practitioner
- Experience/knowledge with Diabetes or sexual health would be ideal
- Educated to degree level
- Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
- Knowledge of Evidence Based Practice and Clinical Governance
- Experience of assessing the needs of and working with individuals with complex health and social care needs.

- Advanced Clinical Practitioner L7 (includes NMP)
