- Working alongside care coordinators, nursing teams and acute visiting clinicians
- Reporting into the PCN Strategic Management team including Clinical Directors

- Strategic Leadership & Change Management – the postholder will lead the development and implementation of the INT model, aligning with ICS priorities, driving integration across system partners, and fostering shared leadership to deliver improved population health outcomes.
- Service Transformation & Pathway Development – they will lead the redesign of frailty and community care pathways, bringing together multidisciplinary teams and system partners to deliver proactive, coordinated, and person-centred care within the local neighbourhood.
- Operational Oversight & Performance Management – as the INT becomes established, the postholder will transition to an operational leadership role, overseeing service delivery, managing workforce and financial performance, and embedding strong governance and performance monitoring.
- Continuous Improvement & Embedding a Learning Culture – they will champion a culture of continuous improvement, using data, patient feedback, and quality methodologies to drive innovation, support learning, and share best practice across the INT and wider system.
- Ideally full-time but may consider a job share – full-time can be worked across 4 or 5 days
- Roughly 50/50 in terms of desk-based and travel to other sites/meetings
- 12-month initial contract with excellent scope for a further 12 months and likely a permanent role beyond that
- High degree of flexibility over working hours
- 6 weeks’ annual leave
