Your career development is a priority, and we will provide ample training and development opportunities to help you reach your targets.What you will do:- Support the assessment, planning, and consistent implementation and evaluation of individualised health care for an identified group of service users within your scope of competency or under the direction of a registered professional, working closely with the multi-disciplinary team to ensure high-quality care.
- Maintain high standards of care by adhering to national clinical guidelines and organisational policies, and challenge practices where necessary to support and guide junior or new staff.
- Ensure continuity of patient care by effectively communicating and liaising with nurses, therapists, and colleagues, providing accurate reports on any changes in service users' conditions.
- Promote choice and enhance the patient and carer experience by supporting service user participation and actively encouraging them to voice their opinions.
- Foster an organisational culture where staff can safely challenge inappropriate behaviour or unsafe practices through proper channels.
- Contribute to the efficient running of the service by maintaining a safe working environment, preserving confidentiality, and occasionally taking on additional reasonable tasks.About you:- You hold a Level 5 Foundation Degree in Assistant Practitioner (Healthcare).
- You come with experience in a healthcare setting, ideally from a specialised service like Neuro or Palliative care.
- Your track record includes effective collaboration within a clinical and multi-disciplinary team.
- You bring knowledge of relevant legislation and its effects on current healthcare practices.
- Your excellent communication and interpersonal skills allow you to build solid relationships with both clients and colleagues.
