What you will do:
  • Conduct comprehensive assessments of patients in their homes or suitable community settings
  • Develop and implement tailored rehabilitation programs focusing on stroke recovery
  • Work collaboratively with multidisciplinary teams to ensure coordinated patient care
  • Engage with patients and their families to set and achieve rehabilitative goals
  • Maintain accurate records and documentation in line with NHS standards
  • Use your car to travel efficiently between appointments and manage your schedule effectively
This role requires a car driver due to the community nature of the work, ensuring fast and flexible access to clients.
