This can be discussed in more detail at interview.Weare looking for an experienced, person-centred Occupational Therapist to join our Adult Social Care Reablement Team in Westmorland and Furness.This is a rewarding role which is central to our Promoting Independence Well-being Programme, which focuses on early intervention and recovery helping people stay in control of their lives and reduce the need for long term care. You will support people to remain independent in their own homes or return home safely after a hospital stay. Youll help individuals regain confidence and live the life they want to, doing the everyday things that matter to them.Main duties of the job
  • What the role involves:
  • Visiting people in their own homes to assess their needs
  • Completing Care Act and functional assessments
  • Creating support plans that promote safety, independence and well-being.
  • Recommending equipment or adaptations to reduce risk and support mobility
  • Working closely with social workers, reablement workers, health professionals and families.
  • Supporting people with a wide range of physical, cognitive, mental health and social needs.

  • Qualifications
  • *Accredited Occupational Therapy Degree or equivalent Professional Occupational Therapy qualification
  • *Registration with appropriate professional body
  • *Evidence of ongoing professional development/CPD within the last 2 years
  • * Evidence of successful completion of, or working towards ASYE
Desirable
  • *Knowledge and application of departmental and council objectives and initiatives
  • *In depth knowledge and understanding of own professional area of practice and issues within Adults Services
  • *Understanding and application of theoretical approaches, models, practices & procedures relevant to customers, carers, professional colleagues
