SOAR is an award-winning regeneration charity in North Sheffield which provides a number of services for the community including health, buildings management and support into employment. We now have an exciting vacancy in our Health & Wellbeing Team for:Dementia Support Worker - Network North Dementia PartnershipPURPOSE OF JOBSOAR Community are seeking to recruit an experienced Dementia Support Worker to engage people with dementia and their carers who are registered with a Network North Primary Care Network GP practice. The aim is to ensuring they feel an increased sense of belonging and connection within their communities, so that they can live well with dementia for longer.The role will:
  • Complete an initial assessment to build trust and ascertain individual needs & wishes
  • Hold a caseload offering one to one support, sharing lived experiences, coping strategies, and guidance to instil hope and confidence
  • Provide encouragement and motivation with regular check in’s for both Client and carer, including a 6-month wellbeing call
  • Support access and navigation to a range of peer support activities available in the community such as dementia cafés, including organising transport to enable regular participation
  • Attend Network North Dementia Multi-Disciplinary Team (MDT) each month
  • Develop close working relationships with Age UK Sheffield, Sheffield Dementia Advice Service (DAS) and Primary Care Networks, etc.
