- Encourage and motivate patients with managing their nutrition.
- Medicines management from an administration chart under the supervision of an Integrated Senior Support Worker.
- The team work from 8am-10pm 7 days a week, shifts are flexible with full and part time opportunities.
- Independent travel between patient's homes and the office base in a timely manner

- National Vocational Qualification (NVQ) Level 3 or equivalent experience
- Satisfactory General Education and evidence of additional training relevant to role.
- Educated to GCSE level
- Previous experience in nursing or social care setting Customer care
- Experience of working independently as well as part of a team
- Previous community nursing and/or Physiotherapy, Occupational Therapy experience
- Experience of working in a community setting
- Basic knowledge of the NSH and healthcare
- Knowledge of basic infection control principles
- Awareness of health & safety issues in the working environment
- Awareness of own skills & the requirements of the role
- Understanding the principles of adult & child protection
- Good standard of general education, including English and Maths.
- National Vocational Qualification (NVQ) Level 3 or equivalent experience
