The team plays a crucial role in helping young people prepare for independence and adulthood. You’ll be supported by a genuinely lovely and approachable team manager and join a warm and well-established team environment.What You Can Expect:Supportive and approachable team managerHybrid working available – flexible around service needsStable, long-term contract with potential to extendFriendly and collaborative team cultureOngoing support and regular supervisionDuties and Responsibilities:Support 14–17 year olds as they transition into adulthood and independent livingCreate and implement tailored pathway plans for each young personWork closely with education providers, health professionals and housing servicesUndertake statutory visits and ensure young people’s voices are central to planningRecord work clearly and maintain compliance with statutory requirementsRequirements:Social Work England (SWE) registrationQualified Social WorkerExperience in Leaving Care, LAC, or working with adolescentsStrong communication and relationship-building skillsAbility to work independently and as part of a teamThis is an excellent opportunity for a committed social worker who is passionate about supporting care-experienced young people and enjoys working in a stable and supportive environment.Contact: Emily Cooper – 07745200325 |
