  • Facilitate smooth and efficient discharges from the hospital, ensuring patients receive the right support when they return home.
  • Collaborate with a multi-disciplinary team, ensuring comprehensive care plans are in place for each patient.
  • Assess patient capabilities and needs, and develop rehabilitation plans tailored to each individual.
  • Provide guidance and support to community staff, care home workers, and families.
  • Evaluate and revise patient treatment plans as necessary to maximize rehabilitation potential.
