- Collaborate with the multidisciplinary team to ensure thorough and holistic assessments for patients being discharged from the hospital.
- Facilitate smooth transitions for patients from the hospital to community settings, ensuring all health and social care needs are addressed.
- Conduct comprehensive assessments in the community, focusing on empowering individuals through a strengths-based approach.
- Provide expert guidance and support to patients and families during the discharge process, offering information and resources to support informed decision-making.
- Work flexibly across various services, including Crisis response, intermediate care, and discharge teams within the hospital setting.
