Are you a Qualified Mental Health Nurse, Social Worker or Occupational Therapist with urgent care experience that is looking to progress and lead on the development of our Home-Based Treatment Team (HBTT)?An exciting opportunity has arisen within Bolton HBTT to work as a Senior Practitioner within a well established Homebased treatment team. The post holder will take on a senior role in responding urgently to referrals from our Mental Health Liaison Service, Inpatient and Community colleagues, offering alternative to inpatient admission for people with complex presentations. The post holder would over see the care of patients under the service along with oversight of patient flow to gain an understanding on where service pressures may be, and proactively support to reduce these within this role in the HBTT. The post holder will proactively support colleagues within HBTT and advocate for people to recover at home in the least restrictive way. As a senior role, there will also be a requirement to supervise and support staff members within HBTT in their professional development alongside participation in service development projects within the team and urgent care services.Main duties of the jobThe post holder will -Focus on ensuring a rapid response to all urgent referrals coming into the service, alongside playing an integral role in supporting patient flow through the system. This will include:Supporting the service to develop and maintain robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions.Proactively identifying and facilitating supported early discharges from our inpatient settings.Supporting with admission avoidance from MHLS and community teams.Supporting the team development to meet the Core Fidelity Standards.
  • Ensure high standards of clinical contact with service users and carers accessing HBTT. This will include
  • Oversee the homebased treatment caseload to ensure service user flow through the HBTT service offer
  • Proactively engage in Senior leadership meetings to ensure KPI and trust targets are achieved
  • Engage in service development projects to enhance care delivery in HBTT
  • Ensure open communication and liaison with community teams via interface
  • Deputise for HBT team manager in there absence
  • Supervise junior members of staff and provide regular IPDR reviews, including following HR guidance around performance
  • Take the lead with all safeguarding concerns & referrals
  • Engage in the locality on call manager rota
