The postholder will be part of the Wessex Clinical Van service, providing outreach treatment pathways for viral Hepatitis across Hampshire, Dorset and the Isle of Wight. This will include health screening and assessment, other health and wellbeing treatment, signposting and onward referral to the relevant services. Our service users are at risk of have having life limiting/life restricting co-morbidities and are at risk of earlier death. This service will aim to increase access to Hepatitis C treatment pathways and other Health and Wellbeing services and play a significant role in identifying illness early with the aim of accessing the right treatment at the right timeMain duties of the jobThis post is for a 12 month fixed term contract.
