What you will do:
  • Meet with service users, carers, families, and multi-disciplinary agencies to enhance independence, safety, and quality of life through rehabilitation planning.
  • Develop and implement individual care and support plans, strategies, and case management.
  • Coordinate assessments and reports, making presentations to relevant multiagency panels.
  • Ensure all work complies with Council policies and procedures and aligns with statutory obligations.
  • Adhere to the College of Occupational Therapy Code of Ethics and Professional Conduct and Health & Care Professions Council Standards of Proficiency.
  • Report to the Team Manager, ensuring workload is managed efficiently and adherence to policies and performance standards.
