- Conducting holistic and person-centred social work assessments of older adults' needs, considering their physical, emotional, social, and environmental circumstances.
- Developing, implementing, and reviewing tailored care plans in collaboration with service users and their support networks.
- Safeguarding vulnerable adults by confidently applying Adult Protection procedures and legislation.
- Managing a complex caseload and prioritising effectively to meet the diverse needs of service users.
- Working autonomously and as part of a multidisciplinary team, fostering effective communication and collaboration with health professionals, housing providers, and other relevant agencies.
- Undertaking duty work, responding to urgent referrals and providing timely and appropriate interventions.
- Maintaining accurate and up-to-date records in accordance with relevant policies and procedures.
- Contributing to the ongoing development of the Older People's Team and participating in team meetings and professional development opportunities.
- A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW).
- Registration with the Scottish Social Services Council (SSSC) as a Social Worker.
- A full driving licence and access to a vehicle.
