We are seeking an experienced social worker to jump into this exciting locum opportunity within our Adults Review Team.   • Undertake assessments of support needs for service users, carers, and families, and assist in screening referrals
  • Develop and review Care and Support Plans and packages to address social care, health, and well-being needs
  • Work collaboratively with service users to empower them through informed decisions
  • Address potential safeguarding issues and follow the relevant policies and procedures
  • Manage risks and promote positive behavioural changes
  • Respond to crisis situations with timely assessments and legal interventions
  • Maintain accurate records and ensure data quality within care systems
  • Advocate for service users, ensuring their voices are heard in meetings and decision-making forums
  • Establish productive relationships with partnership agencies to ensure comprehensive support
  • Participate in ongoing learning, training, and development activities
