- Recognise and escalate issues of concern and/or safety including the deteriorating patient and issues of safeguarding
- Phlebotomy
- Injections
- Wound care
- Bladder (including catheters) & bowel care
- End of life care
- Moving & Handling of patients
- Equipment provision & instruction in use
- Level 3 healthcare qualification
- Knowledge of multi-disciplinary working
- Knowledge of specific role, service & team demonstrated on application
- Experience of caring for someone with a long term condition or at end of life
- Experience of working within a Community Nursing setting
- Experience of using SystmOne
- Able to effectively travel to multiple patient homes and care homes throughout the course of a day including transport of clinical stock required to carry out the treatment
- Sufficient IT skills to learn the systems required for clinical recording, incident reporting and equipment / stock ordering
- Confidence to work out in the community unaccompanied
- Able to work flexible hours to meet service needs including night shifts
- Clinical skills e.g. phlebotomy, bladder & bowel care, wound care, diabetes care, taking vital observations
