- You will be involved in the implementation of clinical care packages, assessment, treatment and the evaluation of patients’ SMS care
- You will support the registered practitioners within the SMS Recovery Hub in implementing an agreed plan of care in association with the patient and in accordance with instructions and training received whilst promoting safe patient care and to be willing to develop and apply new skills to maintain and enhance the SMS clinical service delivery.
- Support sites with the Assessment & discharge arrangements of our complex patients
- Support our patients in HMP Brinsford transition from the Young Offender to Adult estate for continuity of care

- NVQ level 2/3 or equivalent in Health and Social Care
- 1 year experience within SMS (desired)
- Excellent communication skills
- Good IT skills and IT literacy
- Ability to work within a busy environment
- Ability to work unsupervised whilst following policy and procedures
