You will
  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
Lead the development of individualised patients care plans, whilst collaboratively working alongside the inpatient team and championing clinical excellence.Support patients on a daily basis to achieve their activities of daily living to ensure they have the best quality of life possible, ensuring collaborative delivery.Ensure quality care planning and continuity of care is achieved through improving relationships and communication with partners and other services. * Deliver non-judgemental care that makes a real difference, as defined throughout our bespoke Health in Justice competency frameworkIf you are interested in career development, we have a wide range of opportunities for you to develop your skills and experience, which include both internal and external options for development and learning.It doesnt stop there we also offer:

  • NMC registered nurse Adult
  • Evidence of Continued Professional Development
  • Understanding of the complexities of multi-disciplinary and multi-agency working
  • A non-judgmental and compassionate approach
  • Understanding of clinical governance, health and safety, QOF, EBP and national standards
