Job Title: Occupational Therapist
Band: 6
Location: London, Greater London
Working Hours: 37.5 hours per week
Rate: Competitive — details to be discussed upon applicationWe are seeking a proactive and experienced Band 6 Occupational Therapist to join our client in London, Greater London. This role is ideal for someone passionate about rehabilitation and discharge planning within a collaborative, multidisciplinary environment.Key Responsibilities
  • Provide high-quality occupational therapy assessments and interventions for patients on Pathway 2 and support assessments for Pathway 3.
  • Work closely with care home teams, social care, primary care, mental health services, and Continuing Healthcare (CHC) to ensure seamless patient care.
  • Promote a Home First ethos, aiming to support patients in returning to their original place of residence whenever possible.
  • Where discharge home is not feasible, conduct holistic assessments to determine the most appropriate onward placement in collaboration with patients and their families.
  • Contribute to discharge planning and rehabilitation goals, ensuring timely and safe transitions of care.
  • Maintain accurate and timely documentation in line with professional and legal standards.
Qualifications & Experience
  • BSc (Hons) or Diploma in Occupational Therapy
  • HCPC registration as an Occupational Therapist
  • Experience in rehabilitation, discharge planning, or community-based care
  • Strong communication and coordination skills within multidisciplinary teams
  • Understanding of the Home First model and discharge to assess pathways
