What you will do:
  • Work collaboratively with the team manager to support the locality team.
  • Engage with individuals over 18 in the community to complete Care Act Assessments.
  • Incorporate the Mental Capacity Act and DOLS while assisting with court work.
  • Support with Continuing Health Care Assessments to ensure comprehensive care provision.
  • Be proactive in identifying needs and developing care plans in collaboration with other professionals.
