- Positively impact the public health of patients and residents in Tower Hamlets via immunisation programmes for adults and children, national screening programmes, health promotion.
- Providing education and holistic self-management support for patients with complex and long-term conditions, (for example diabetes, asthma, COPD, Hypertension, Chronic Kidney Disease) including consideration of their mental health and social circumstances, enabling them to stay well and out of hospital.
- Supporting families and carers who are caring for those with mental or physical problems and distress.
- Maintaining and improving the excellent reputation of Tower Hamlets as a caring and learning community of practitioners by working in partnership.
- Be a crucial part of delivering the 10 year Health Plan for the Future FIT FOR THE FUTURE
- To maintain own annual registration with the NMC and professional indemnity and adhere to The Code: Professional standards of practice and behaviour for nurses, midwives and nursing associates
- Ensure all statutory regulations (e.g. Health & Safety, COSHH, Infection control.) are implemented and adhered to
- To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
- To maintain own continuous professional development (CPD) and ensure CPD is kept up-to-date through annual appraisal in line with the requirements of the Practice and The Code
- Emergency Equipment maintenance and audit

- Educated to degree level obtaining First Class/2:1/2.2qualification BSc or hold MSc or have studies a Level 7 module within the last 5 years
- Understanding and evidence of equality, diversity & inclusion within the health care setting.
- Knowledge of primary health care General Practice and the role of the General Practice Nurse
- Demonstrate flexibility towards and understanding of the processes involved in gaining new clinical competencies, moving from novice to expert
- Demonstrates awareness of importance of working as part of a multidisciplinary team
- Registered with the Nursing & Midwifery Council
