  • Oversee same-day discharge and admission prevention.
  • Ensure the safe and timely discharge from the acute floor.
  • Conduct Care Act assessments with precision and empathy.
  • Apply your safeguarding knowledge to protect vulnerable patients.
  • Work collaboratively within a multidisciplinary team in a fast-paced environment.
  • Address patients’ mental capacity assessments (MCAs) as needed.
  • Provide expert guidance and support to patients and their families.
