We are recruiting for a Frailty EAAR (Early Alternative to Admission Review) Clinical Nurse Specialist who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.We are a multidisciplinary team comprising Consultant Geriatricians, Advanced Clinical Practitioners (ACPs), Clinical Nurse Specialists, and a Frailty Lead Pharmacist. Our team operates a seven-day service based in the Emergency Department (ED) and Acute Medical Unit (AMU), providing specialist assessment and care for frail and vulnerable patients.As part of the Integrated Care Board's (ICB) initiative to reduce unplanned hospital admissions by 25%, one of the key interventions within the acute trust is the Early Alternative to Admission Review (EAAR), conducted upon patients' arrival to the hospital.This role involves clinically assessing frail patients who present to the Emergency Department, serving as a secondary triage to identify and direct suitable patients onto alternative care pathways at the point of arrival.For patients not appropriate for the EAAR pathway, the focus will be to initiate a comprehensive geriatric assessment (CGA) aimed at minimising duplication, reducing time spent in the ED, and improving patient outcomes--supporting a "home first" model of care.At the heart of everything we do are our core values:Include,Respect, andImprove.
