Contract Type: Part-time, PermanentAbout the Role:An excellent opportunity has arisen for a Approved Mental Health Professional to join the Frontline & Community Team in Surrey. This vital role involves conducting Mental Health Act assessments, ensuring the rights and safety of individuals, and working collaboratively with multi-disciplinary teams to provide high-quality, person-centred care.You will play a key role in safeguarding individuals’ well-being and promoting recovery, while ensuring compliance with relevant legislation and professional standards.Key Responsibilities:
- Lead on and complete holistic assessments of the social care needs of individuals by working in line with the Integrated Care and Health Pathway and in partnership with individuals, carers, professional staff in other agencies and service providers to establish arrangements and services that meet their assessed needs and promote independence, choice and control.
- Manage a caseload of individuals and carers who require additional support or guidance in order to identify their needs and who may need close monitoring or frequent review or interventions to minimise and manage risk, or where best interest decisions may be required.
- As practitioners within the teams, the AMHP is expected to understand the recovery model and promote this way of working with people who use services/carers. They are also expected to promote this model of working with health colleagues and support their practice when necessary.
- Carry out assessments and reviews of individuals, carers and families who need specific support, protection or advice to ensure that their needs are met and they are safeguarded.
- Have direct contact with the public by telephone and visiting them in their own homes or care settings, across a wide geographical area.
- AMHPs will be expected to work with any combination or all of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning disability, and people with mental health needs.
- Promoting independence, choice and control is at the heart of this role. Therefore role holders will work in a person-centred way proactively seeking to empower individuals, their families and carers to maximise their potential for independence.

- A recognised Social Work qualification (e.g., Degree in Social Work, CQSW, DipSW, or equivalent)
- Current registration with Social Work England
- Post-qualification training in areas such as transition planning, safeguarding, or adult social care (desirable)
- In depth and up to date applied knowledge of relevant national and local policy, statutory guidance and legislation in relation to the provision of social care services.
- Understanding of the principles of confidentiality and information governance and how these apply to social care.
