- To undertake direct work with non complex or post 18 individuals and take the lead role in reviewing young people's Pathway Plans and Risk Assessments.
- To carry out actions as outlined in the Pathway Plan and be involved in the preparation, co-ordination, and review of the Pathway Plan in accordance with required standards and timescales.
- To provide a comprehensive and personal support to all assigned cases and carry out practical tasks relating to all aspects of the Pathway Plan including:
- Provision of an intensive housing support service to young people with complex support needs, developing creative ways in which to engage them.
- Develop effective working relationships with partner agencies and colleagues in accommodation - for example supporting partners, such as Vulnerable Adults, local letting agents and other accommodation provider's district and regional wide in planning and co-ordinating suitable and safe accommodation.
- To develop and deliver a range of methods and approaches that enables care leavers to secure and maintain accommodation appropriate to their needs.
- To provide practical housing-relating support to young people, including budgeting, claiming benefits, maintaining utilities payments, equipping and maintaining the physical standard of the accommodation.
- Delegate relevant aspects of the work accordingly to the specialist roles within the service and external agencies as appropriate
- To achieve good liaison, communication and networking within the local authority, statutory and independent agencies and local communities, ensuring an effective cohesive service for all children and young people.
- To work directly on an individual basis or in groups to deliver services for assessed needs.
- To prepare and maintain records and documentation related to the work using IT systems for this and be accountable for that work.
- To attend and participate in Looked After Children reviews /CAF/CIN and other meetings which concern children and young people in care or who have left care.
- Knowledge and understanding of the difficulties young people face in making the transition from adolescence to adulthood.
- Knowledge and understanding of the Children (Leaving Care) Act 2000 and other related legislation.
- Experience of working with young people in the care system.
