An exciting opportunity has arisen for an experienced and highly motivated Registered learning Disability Nurse (RNLD) Registered Mental health Nurse (RMN) or Registered General Nurse (RGN) to work as a CHC Nurse Assessor for Gloucestershire ICB.We are looking for an individual who has excellent assessment and communication skills. The successful applicant will need to demonstrate the ability to work both as part of a team and also be able to work autonomously across agencies.As a Band 6 CHC Nurse Assessor you will be responsible for determining the eligibility for NHS Continuing Healthcare funding through new assessments, review assessments and appeals, in accordance with the National Continuing Health Care Framework. The role at times involves case management of individuals that are eligible for NHS CHC funding.Main duties of the jobNurse Assessors are expected to:Undertake nursing care assessments including Continuing Health Care (CHC) assessmentsReview Funded Nursing Care (FNC) and CHC packagesHave a knowledge of the CHC FrameworkBe highly motivated and keen to learn/develop (training in respect of the NHS CHC Framework and its application will be provided as part of induction);Be accustomed to a high degree of autonomy on a day to day basis;Have excellent verbal and written communication skills, with attention to detail;Have excellent people management skills with experience of difficult situations and challenging individuals and the ability to manage these effectively and calmly;Be comfortable working with a wide variety of ages and individuals with very different clinical conditions.Have excellent time management skills and thrive on working at pace.Have excellent knowledge of Microsoft office and have intermediate typing skills.Be comfortable with changes at short noticeBe able to travel daily across the countyAbout usAt our Organisation, our selection process ensures we recruit candidates with the right skills and values.   • Relevant Degree/evidence of being able to work at that level.
