- RN/ RMN 1st level.
- Up to date NMC registration.
- Relevant Degree/evidence of being able to work at that level.
- Experience of caseload management.
- To be able to manage a caseload of patients with complex needs.
- Ability to undertake complex nursing assessments.
- Able to monitor high standards of care.
- Able to direct and co-ordinate a programme of work, collaboratively or autonomously.
- Knowledge of legal issues regarding NHS funding.
- Fully aware of NICE guidelines for the provision of specialist equipment and Long Term Conditions.
- Knowledge of promotion of continence.
- IT experience.
- Highly developed communication skills, both verbal and written, including the writing of comprehensive reports.
- Audit experience.
- At least three years registered nurse experience.
- Working knowledge of the National Framework for CHC and FNC.
- Working knowledge of Primary Care Services.
- Palliative Care experience.
- Community/ Care Home experience.
- Evidence of continuing professional development at degree level.
- Community Nurse Qualification.
- Teaching and assessing-998 or equivalent.
- ECDL.
- Continence course.
- Dementia course.
- Tissue Viability course.
- Ability to fulfil the physical requirements of the post.
- Must be proficient in clinical skills required for area of practice including knowledge of equipment and continence products.
- Ability to travel to all areas of Gloucestershire and occasionally out of county.
