- Conduct assessments and develop care plans tailored to the individual needs of young adults with learning disabilities.
- Support young adults in accessing necessary services, promoting independence and well-being.
- Collaborate with families, caregivers, and other professionals to implement the best care strategies.
- Ensure compliance with the Mental Capacity Act (MCA) and the Care Act, applying relevant legislation in all interactions.
- Complete and attach the MOC cover form to ensure comprehensive and up-to-date case records.
- Travel within the community when necessary, so a valid driving licence is required.
