Join us and contribute to making a positive impact on our community.The RoleWe are seeking a dedicated and experienced registered Social Worker to join our Hospital Discharge Assessment Team (HDAT). HDAT provides adult social care support, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005, to residents of North Somerset who have ongoing social care needs and are being discharged from hospital after an in-patient stay. We follow the Hospital Discharge and Community Support guidance.The team consists of Senior Social Workers, Social Workers and Adult Social Care Workers. Staff undertake Care Act assessments, and mental capacity and best interest assessments. Staff identify eligible needs and produce care and support plans to ensure a person's needs are met effectively for the longer term. Staff advocate for the individual whilst working positively with family members and carers, together with other professionals involved with the person, for the best outcomes. Staff work proactively in a strengths-based way to ensure people are also able to draw on their own abilities and support networks for a quality of life, in addition to care provision. Staff work hard in a fast-paced environment to support timely discharge with minimal days lost. Assessments are carried out in the person's home, in a D2A pathway location or in hospital. Social Workers in HDAT also have a presence in our acute hospital Transfer of Care Hubs.This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.Work StyleAt North Somerset Council, we offer a hybrid working environment based on four distinct work styles.
