What you will do:
  • Manage a clinical caseload of individuals with substance misuse issues, providing holistic, person-centred care and support.
  • Conduct comprehensive assessments, including risk assessments, and develop, implement, and review care plans tailored to individual needs.
  • Plan and deliver community-based detoxification programmes, including alcohol and opiate detox, supporting safe medication management such as opiate substitute treatment.
  • Promote harm reduction strategies, including naloxone distribution and Blood Borne Virus (BBV) screening and vaccinations.
  • Work collaboratively with mental health, physical health, social care services, and third-sector partners to ensure integrated care pathways.
  • Provide clinical leadership and supervision within the team, supporting colleagues and acting up for the Team Leader during short-term absences.
  • Maintain accurate, confidential clinical records in line with professional standards and organisational policies.
  • Facilitate and co-facilitate group sessions aimed at recovery and health promotion.
  • Communicate effectively and empathetically with service users, families, carers, and professionals, maintaining a non-judgmental approach.
  • Contribute to service development, monitoring, and evaluation to improve care delivery and outcomes.
Qualifications and Registration:– Registered Nurse (RGN or RMN) with current NMC registration.
– Degree or diploma in substance misuse or equivalent qualification desirable.
– Evidence of KSF/DANOS competencies relevant to the role.Experience:– Substantial experience working in substance misuse or mental health nursing, preferably in community settings.
– Experience managing complex caseloads involving co-occurring mental and physical health conditions.
– Proven ability to work autonomously and as part of a multidisciplinary team.
