- Serve as the first point of contact for patients with undifferentiated or complex conditions via face-to-face, telephone, or acute on-the-day appointments.
- Assess, diagnose, plan, implement, and evaluate care for patients with acute, chronic, or complex needs.
- Order, interpret, and act on diagnostic tests and investigations.
- Admit patients to secondary care or refer to specialties when clinically indicated.
- Take autonomous clinical decisions within your scope of practice.
- Prescribe, review, and monitor medication safely as an independent prescriber, supporting patient adherence.
- Work collaboratively with the multi-disciplinary team, providing clinical leadership and contributing to service improvement.
- Registered with a UK professional body (NMC, HCPC, or equivalent).
- Degree-level registration qualification or equivalent post-registration award.
- MSc in Advanced Practice or equivalent qualification, or willingness to undertake.
- Life support qualifications.
- Non-medical prescribing qualification.
- Advanced clinical practice skills and experience managing patients with long-term or complex conditions.
- Ability to recognise and work within professional competence and codes of conduct.
- Substantial experience working in Primary Care.
- Significant post-registration experience.
- Work in a modern, accessible health centre serving approximately 10,000 patients in Seafield, Aberdeen.
- Flexible part-time working with protected clinical and administrative time.
