  • Develop and implement tailored discharge plans that address each patient’s unique needs.
  • Collaborate with a multidisciplinary team to ensure seamless transitions and continuity of care.
  • Provide expert advice and support to patients, families, and carers during the discharge process.
  • Maintain accurate records and documentation in line with professional standards and regulations.
To succeed in this role, it is essential that you are HCPC registered and a confident driver to facilitate community visits and home assessments.
