We have a fantastic opportunity for a part time band 5 nurse looking to move into the field of Community LD Nursing to join Camden Integrated Learning Disability Service working with people with learning disability in their community to lead fulfilling lives .The TeamCamden Learning Disability Service is a integrated multidisciplinary service consisting of community nurses, social workers, physiotherapists, occupational therapists, speech and language therapists, specialist support workers, accessible information officers, psychologists and psychiatrists.CLDS works in partnership with other agencies across the borough to support people with a learning disability with their physical and mental health needs.The position will provide the opportunity to develop skills in specialist Learning Disability nursing, mentoring students, pathway working and multi-disciplinary work whilst being part of a supportive team.We will provide regular clinical supervision and you will be supported to continue your professional development.Main duties of the jobTo work as an integral member of the Community Nursing Team for People with Learning Disabilities. To assess the care needs, design, develop and implement and evaluate programmes of care in partnership with clients and carers. To provide clinical advice/support to people providing services to the client group as a member of the wider Integrated Learning Disabilities Service. To assist in developing services. To liaise appropriately with other agencies. To maintain a designated caseload.
