We currently have a number of exciting opportunities to join the All Age Continuing Care Nursing and Case Management teamsThese posts cover all of NHS continuing care including adults, children and complex case management.Please note we reserve the right to close this vacancy prior to the closing date once the required number of suitable applications have been received.Main duties of the jobThere are a variety of positions including case management and nurse assessor positions at band 6. Roles will be ICB wide covering North and South of the geography, with bases at the nearest ICB base to home.These roles will be fast paced and interactive with patients, families, clinicians and other partner agencies so will require good communication skills, attention to detail and teamwork approach.Training on the appropriate software will be given.About usOur Integrated Care Board (ICB) is the largest in the country.The ICB is responsible for ensuring that high quality and safe health services are accessible to all of our communities. To undertake reviews in line with the National Framework for Continuing Healthcare and Funded Nursing Care To process Local and Independent requests for appeal against NHS Continuing Healthcare eligibility decisions for the ICB To undertake reviews for Previously Unassessed Periods of Care (PUPOC) on behalf of the ICB. To undertake Retrospective reviews on behalf of the ICB. Complete the Appeals process within set timescales. Completion of the individuals Care Needs Portrayals in order to inform the Appeal process. Respond to NHS England data requests in a timely manner.
