- Responsible for supporting referrals for the area you are assigned to, carrying-out comprehensive assessments and developing perinatal specialist mental health care plans.
- They will monitor and support women during the pregnancy or at post-natal stages, maintain strong links and communication with health providers such as Community Midwives, Specialist Midwives, Health Visitors and other professionals.
- Planning care programme approach (CPA) reviews.
- Required to attend and cover key functions in the team,- duty, team huddles, team meeting, buddy support/lone working.
- As a member of a large team, you will provide support to your colleagues with complex cases, escalating concerns to Safeguarding partners and ensuring that safety plans are in place for high-risk service users.
- To keep up to date with specific specialist perinatal research.
- Undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
