We are seeking a compassionate and organised Care Coordinator to join our reception team in this vital front-of-house role. As one of the first points of contact for many patients, you will play a key role in improving access to care by helping to guide patients quickly and effectively to the right professional or service.With comprehensive training, you will support the clinical triage by helping to identifying the most urgent cases and ensuring they are prioritised appropriately. You will also provide essential assistance to patients with additional needs or those requiring reasonable adjustments, helping to ensure this information is clearly visible on the clinical system and online consultation tool to enable the practice to provide an inclusive, and supportive environment for all.Your excellent communication skills and empathy will enable you to understanding patient needs and directing them with confidence. Working closely with the duty doctor and the reception team you will contribute to the smooth running of the practice and help enhance patient experience.If youre a team player who thrives in a busy environment and wants to make a real difference in peoples access to healthcare, we would love to hear from you.Main duties of the jobAs a Care Coordinator, your main responsibility will be to support patient access by managing appointment requests and ensuring they are allocated appropriately in line with the practices clinical triage protocol. You will carefully review incoming appointment requestsboth online and by phoneensuring sufficient information has been gathered to allow safe and effective triage by the clinical team.You will be responsible for organising and booking both urgent and routine appointments, balancing clinical need with appointment availability. Using NHS tools such as NHS Choices and local service directories, you will also help direct patients to the most appropriate service or support, which may not always be a GP appointment. This helps to improve access, manage demand, and empower patients with the right information.Working as part of the reception team, you will play a key front-line role in supporting patient care, improving access, and helping to deliver a positive patient experience. There may on occasion be a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.
  • Personalised care navigation for patients contacting to support access to care
*Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition. This may mean contacting patients who have requested an appointment to advise of the most appropriate service for their issue.*Review requests for appointments and contact patients to request more information if required to support triage of appointment requests.*Process patient requests for appointments and book appointments ensuring the patient remains informed. Booking appointments and keeping the patient informed.*Manage day to day allocation changes owing to absence or changes where applicable. This is currently via askmyGP and the care coordinator would update daily allocations due to home visits or changes in the rota on the day.*Resolving any issues in relation to workstreams and ensuring both patient and practice are kept informed of progress*Co-ordinating long-term condition annual reviews in conjunction with the medicines management team, for more vulnerable groups, ensuring a personalised care approach.*Raise awareness of health promotion and NHS health checks in practice and support national screening programmes such as cervical screening and childhood immunisations.
  • Bringing together patients identified care and support needs, including any reasonable adjustments and exploring their options to combine these into a single personalised care and support plan (PCSP), in line with PCSP best practice. Ensuring this is accurately coded to the health care record and reasonable adjustments are readily available to the wider team.
  • Raising awareness of shared decision-making and decision support tools and assist peopled to be more prepared to have a shared decision-making conversation.
  • Ensuring that people have good quality information to help them make choices about their care.
  • Assisting people to access self-management education courses, peer support, interventions & services, including NHS digital platforms, eg NHS App, that support their health and well-being.
  • Supporting residents in care homes/LD homes ensuring personalized care is delivered through collaborative working between health, social care, voluntary, community and care home partners.
*Have a good understanding of the electronic referral system and support the practice and the patient with referral queries. Liaising with colleagues regarding patient referrals and any follow up needed.
