This full-time role is based mainly on “The Limes” ward within St James Hospital – Milton.   • Assess, plan, and implement care plans for individuals ready to be discharged from hospital
  • Coordinate with health professionals, families, and community resources to plan discharges
  • Source appropriate community placements or support networks for individuals
  • Maintain detailed records and documentation on patient progress and plans
  • Attend and contribute to multi-disciplinary meetings to discuss patient’s needs
  • Carry out placement visits which will require a driving licence and own vehicle
