- Degree in Pharmacy, Registration with the General Pharmaceutical Council (GPhC), Postgraduate Diploma in Clinical Pharmacy or equivalent, Evidence of commitment to and able to demonstrate CPD
- MSc in Clinical Pharmacy or equivalent training, Proven post graduate experience as a clinical specialist pharmacist, Completed Independent Prescribing course
- Understanding of commissioning process and delivery of health care within new NHS structures / organisations (Implications of NHS changes on delivery of care), Knowledge and skills in applying medicines legislation in health care and/or social care settings, Excellent knowledge and practice of clinical pharmacy, Understanding of Clinical Governance and Risk Management, Understanding of teaching / learning styles and methods, Good broad clinical knowledge, Good IT skills and knowledge of pharmacy computer systems
- Understands relationship between health & social care, Knowledge of pharmaceutical needs of patients and medicines optimisation issues, Knowledge of pharmaceutical needs of intermediate care services, Demonstrates an awareness of interface issues, Experience of electronic prescribing, Good presentation skills, Appraisal skills, Good trainer, Ability to use research findings to develop clinical services and guidelines
- Extensive post registration experience in homecare and high cost drugs, Proven track record of postgraduate experience in hospital pharmacy, Experience /understanding roles of other health care professionals e.g. staff working in community / multidisciplinary teams, Experience of managing, supervising and training of staff
- Experience of implementing national policies or initiatives e.g. NPSA alerts, Experience of MI and Formulary Management, Experience of providing pharmaceutical support to innovative services, Experience of supporting decision making in complex situations, Evidence of activity in research or audit, Evidence of service developments, outcomes and publications, Experience of formulating policies & procedures
- Excellent critical appraisal skills, Evidence of excellent communication skills (verbal and written) with healthcare professionals and patients, Evidence of good clinical pharmacy practice, Evidence of successful relationships with multidisciplinary teams and across professional groups, Good prioritising and organisational skills, Effective time management skills and delegation, Computer literate, Ability to use own initiative, Demonstrates an appropriate level of clinical reasoning and judgment Ability in problem solving
