Amount to be repaid in full should they leave within two years of employment.Who Cares? We do.An exciting opportunity has arisen for a full-time Team Manager in the Central Home first Hub. We are looking for a manager who is highly motivated, experienced and with a passion for social work.The main duties and responsibilities as a Home first Team manager will be manging the team that supports patient discharge from our Discharge to Assess pathway 2 intermediate care beds located at Norfolk Community Health and Care NHS Trust sites across the central belt. Your base will be at Norwich Community Hospital, however, you may be required to travel the wards located in the North and South localities on occasion. A sound knowledge of the Care Act and supporting planning is essential as well as strong safeguarding adults knowledge. Any applicant is required to have a belief in the benefits of integration to patients and the wider health and social care system. This role sits within an experienced integrated management team with a skill mix of nursing, therapy and social work.You need to demonstrate a commitment to a multi-disciplinary approach and evidence your contribution to multi-agency working. Intermediate care discharge is diverse and complex, but always rewarding and interesting. You will need to have a strong knowledge of social care legislation and processes and the ability to work at pace.
