We are looking for a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. As a key member of the surgery, you will play a vital role in delivering high-quality, patient-centred nursing care.The role of the Practice Nurse is central to modern general practice supporting patients in managing long-term conditions, providing health promotion and preventative care, and delivering essential treatment room services. You will work closely with GPs, other nurses, and the wider multidisciplinary team to improve health outcomes, reduce health inequalities, and ensure patients receive safe, effective, and personalised care.This is an exciting opportunity for a nurse who is passionate about making a difference, committed to professional development, and keen to contribute to the ongoing improvement of primary care services within the NHS.Main duties of the jobThe core responsibilities of the Practice Nurse are as follows. On occasion, there may be a requirement to carry out tasks not listed below, that are within the reasonable scope of the role.To support the Infection Prevention Control (IPC) lead and to work to the IPC PolicyTo assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practiceImplement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeingTo identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients healthProvide routine nursing care to patients in accordance with clinical based evidence, NICE and NSFTo diagnose and manage acute and chronic conditions as requiredTo review patient medications to enhance complianceCo-ordinate the provision of travel medicine servicesMaintain accurate clinical records in conjunction with existing legislationEnsure read codes are used effectively by all member of the nursing teamEnsure appropriate use of the chronic disease registersPrioritise health issues and intervene appropriatelyTo support the team in dealing with clinical emergenciesRecognise, assess and refer patients presenting with mental health needsTo implement vaccination programmes for adults and childrenAbout usHigh Street Surgery is a well-established, forward-thinking GP practice delivering high-quality care to a diverse patient population of approximately 12,000.   • Experience using GP clinical systems (SystmOne).
  • Qualification in chronic disease management or minor illness.
