- This role will include working with the locality-based Clinical Leads under the guidance of the Clinical & Strategic Lead. Excellent opportunities will be available for training and professional development, with regular supervision and access to other support.
- This PCN CYP MH Practitioner posts are a new service to support the clinical development and delivery of the Primary Care Networks (PCNs).
- Post holders would be community-based spending time in primary care settings and other sites that supports access.
- Low intensity interventions offered for early intervention cases.
- Triage and escalation.
- Signposting.
- We are looking to recruit an applicant with expertise in the delivery of therapeutic interventions to children and young people (CYP) and their families to manage anxiety, low mood, and behavioral problems.
- The post holder would be expected to work closely with the Service Manager and Clinical lead. They would also be expected to participate in the training opportunities available to them.
- To work as part of a multi-disciplinary team in a patient facing role to assess and manage patients using expert knowledge.
- To assist in the assessment of new patients with subsequent production and completion of individual care plans.
- To provide specialist clinical support and advice to young people with mental health difficulties.
- To work with patients to support engagement with appropriate services and treatment which may include onward referral to secondary mental health services or signposting to other mental health services, voluntary services and support groups.
- To educate and involve family members and others in treatment as necessary.
- To be responsible for the day-to-day planning of personal workloads.
- To follow departmental policies procedures and guidelines.
- To liaise with the practices and, where practicable, to standardize referral processes across the PCN.
- To triage and also receive referrals and directed patients from within the PCN.
- To communicate effectively with patients, recognizing the need for varying methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
