An exciting opportunity has arisen for a Community Practitioner to join our new Intensive Support Team (IST) working alongside the Community Autism and Learning Disability Service.This is a community-based, specialist team preventing crisis, reducing hospital admissions, and supporting people in the least restrictive setting.The service will be integrated at a local level and provide psychological and psychiatric person-centred interventions.The team is intrinsically linked with the dynamic support register and offers support over the 7-day period and aims to proactively target people with learning disabilities and/or autism who are most at risk of unplanned admissions due to mental illness or behavioural presentation.The Pathway Manager is responsible and accountable forMonitoring the performance and quality of the service the team delivers,Clinical leadership and effective day-to-day management of the team,Commitment to promoting recovery and maximising independence and fulfilling wider management responsibilities as required by the organisation and performance management .We are seeking an individual who has experience and knowledge of supporting individuals with a learning disability and autism.The role offers the opportunity to influence service development and play a key part in the shaping how we deliver community care to people with Learning Disabilities and/ or Autism.Main duties of the jobProvide day-to-day management of the team, hold excellent communication skills and ability to form positive working relationships with Systems PartnersSupport with recruitment, retention, and induction of staffConducting clinical and managerial Supervision, Appraisals, and oversight of individuals developmentManaging performance and absence/staff wellbeingEnsure statutory and mandatory training is completedSupport implementation and development of patient and carer experienceOversight of a support with Informal and Standard Complaints processEngagement within Interface MeetingsDealing with grievance and disciplinary casesMotivate, engage, and develop the team within a multi-disciplinary team environmentLead and coordinate team communication, including holding regular team meetings, communicating, and consulting on service changes (which may be contentious), and acting as a conduit of informationSupport Team following Serious Incidents and with Debriefs, Local and After-Action Review Process and Serious Incident PanelsSupport staff offering learning and training opportunities, clinical supervision, and a supportive, collaborative team culture.About usWe aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future.   • First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification e.g. Social Worker, OT, Clinical Psychologist, other.
  • Relevant Post Graduate qualification or Training.
Knowledge and ExperienceEssential
  • Experience of coordinating and managing staff resources
  • Experience of working within a multi-disciplinary team using a community based approach.
  • Experience of providing community based mental health services.
  • Experience of leading teams and taking on a leadership role or project management.
  • Commitment to Partnership working both multi-agency and multi-disciplinary.
  • Coordinating and supporting staff in the day to day provision of clinical services.
  • Experience of Performance Management
  • Awareness of equal opportunities legislation and policies.
  • Experience of collaborative working across agencies and professional disciplines.
  • Proven ability to manage a clinical caseload in a flexible manner as required
  • Experience of providing clinical/managerial supervision to clinical staff.
