We are excited to be expanding our Dementia Support Services and are looking for two enthusiastic and compassionate Dementia Support Workers to join our team. Each post is for 15 hours per week, with flexible scheduling to suit the needs of the service and team.The post holder will be responsible for the planning, implementation and delivery of our activity groups that we provide in various locations across north Northumberland with the support of your line manager. The post holder will work within a small team which is currently expanding and will include supporting clients and carers at home, or on the telephone.   • Working in partnership with other providers of Dementia services to enhance provision and accessibility of services
  • Assist line manager to Identify and recruit a volunteer workforce to enhance Dementia service delivery

  • Be aware of Hospice procedures relating to identification of safeguarding incidents and vulnerabilities of patients with Dementia
  • Identify service development opportunities for patients with Dementia and engage carers in the development of services
