
Dementia Support Services support worker

- Promotion within local North Northumberland communities of HospiceCare North Northumberland Dementia services
- Undertaking risk assessments prior to patients/ clients undertaking Dementia activities to ensure risk of injury is minimised
- Working in partnership with other providers of Dementia services to enhance provision and accessibility of services
- Assist line manager to Identify and recruit a volunteer workforce to enhance Dementia service delivery
- Planning and implementing activities such as crafts, and music events.
- Be aware of Hospice procedures relating to identification of safeguarding incidents and vulnerabilities of patients with Dementia
- Sourcing activities and members of the community with skills to share at our cafes, including contacting schools and youth groups.
- Leading activity groups that we provide including seated exercise class, 3 cafes and walking football in 2 locations.
- Understand the diagnostic pathway and progression of Dementia
- Understand the therapeutic approaches and relative value of treatment options available for patients with Dementia
- Understand the impact of common health problems which may impact quality of life of patients with Dementia
- Be aware of Hospice procedures relating to identification of safeguarding incidents and vulnerabilities of patients with Dementia
- Identify service development opportunities for patients with Dementia and engage carers in the development of services
