Role Purpose:To deliver high-quality social work practice within the Mental Health Team, supporting adults with complex mental health needs. This includes assessments, care planning, and interventions to promote recovery, independence, and safeguarding.What You Will Do:Assessment and Intervention:
  • Conduct comprehensive assessments of adults with mental health needs in line with the Care Act 2014 and Mental Health Act 1983 (as amended).
  • Develop and implement individualized care and support plans to promote mental well-being and recovery.
  • Undertake mental capacity assessments and support best interest decision-making when required.
Case Management:
  • Manage a complex caseload of adults with mental health conditions, ensuring timely interventions and effective service delivery.
  • Maintain accurate case records and documentation in compliance with statutory guidelines and local policies.
  • Monitor and review care plans regularly, ensuring they remain responsive to changing needs.
Safeguarding:
  • Identify, investigate, and respond to safeguarding concerns, ensuring the implementation of appropriate protection measures.
  • Participate in multi-agency safeguarding processes and contribute to safeguarding enquiries.
